Our Mission

The Bellview Hospice Care Inc. mission is to provide holistic compassionate care to the population we serve and to their families facing uncertainty. The patient maximum level of comfort is our outmost priority. To achieve these our dedicated team will actively be involved in the physical emotional and spiritual aspect of every single patient and family that we care for.

WHO IS ELIGIBLE FOR HOSPICE CARE ?

  • Hospice is for patients who have a life limiting illness in advanced stages and needs supportive treatment and care. Diagnosis’s may include end-stage lung, kidney, heart or neurological disease, or cancer.
  • Hospice is for the patient who is no longer undergoing curative treatment.
  • Hospice is for the patient with a life expectancy of six months of less. Hospice is not only for patients who are actively dying, but also for patients who wish to live out their remaining days/weeks/months on their terms: pain free, with family & loved ones in an environment that allows for the patient and family to cope with the realities of the disease.

Bellview Hospice Care Inc. Team

Our Team members specialize in caring for the hospice patient and supporting the patient’s family and loved ones.

Every hospice patient is assigned a team of providers. The hospice team is made of professionals that include:

Trained Volunteers - Provide companionship and support to patients and families. Volunteers visit with patients and/or provide respite for a caregiver while they run errands.

Spiritual Counselor - Available to provide nondenominational spiritual support directly or can act as a resource by contacting local clergy.

Bereavement Counselor - Provides information and intervention for support including crisis intervention and provide individual or family support.

Medical Director - Leads the team in developing a plan of care and managing symptoms related to the hospice diagnosis.

Registered Nurse - Coordinates services and care with the hospice physician and oversees patients care.

 

Licensed Vocational Nurse - Assist the registered nurse and makes emergency visits when necessary.

Social Worker - Assess the needs of the patient and family, provides resources and advocates on the patient/family’s behalf.  Hospice social workers are trained, licensed professionals who help patients and families address many types of social, emotional, and practical challenges near the end of life.
